I don't get it. Just like the bonus says, why is the counselor a dentist?

The DDS bit is just Weinersmith accidentally using the wrong degree. This is one of the older comics where the bonus panels were drawn by him going back to old comics after over a decade, so he's just drawing himself as confused as to his previous mistake.

The actual panel isn't that deep I think. The guy is dumb and doesn't realize that offering to pay for her facelift implies his wife is unattractive and needs a facelift.

Your bot is putting the date into the alt text

That's the way it is on the original 🤷 Maybe he forgot to update those comics? He added the alt text and bonus panels for these old comics like a decade after they were originally written.

EDIT: Looks like he just stopped coming up with alt text for the older comics after this. All of the old comics I checked after this have an alt text of just the date up through https://www.smbc-comics.com/comic/2015-03-03, which is the last one that's titled by day as well. I might just stop posting the alt text for these comics. Also it's the title attribute, not the alt attribute, but now it's too much work to go back and change all of the old posts. Oh well.
