I bet they were rushed by execs, there's a pattern I'm aware of of engineer-types wanting to do more testing/take more time to do it right, and execs wanting to cut corners. I do get keeping resource use down, triage, there isn't infinite money for everything, but you probably should take the time and money to test if a thing works, or in the case of bridges or mission-critical flight/medical software, will be safe, or you get a story like this and nerds like myself criticizing you online for repeating history. Also very deserved lawsuits if it is one of those life-critical things and not just a fun game.
