Don't most dogs "go" inside a house before they're housetrained? How do you then train them to not "go" in a store or mall without them failing at least a few times?

Also, the mall around my place is HUGE and people stay in there for hours. Do dogowners take bathroom breaks for their dogs outside every two hours? What do you do with your dogs bathroom needs for all that time?

Riffing with this comment, I usually train them on grass and dirt. I also sometimes will leave some poop (initially) in the designated spot so they can associate that area with the bathroom. First thing in the morning is the best time because they need to go and you can easily bring them to the appropriate spot.

This is why it is critical you DON'T let them poop/pee in places you don't want because they will associate that with the bathroom, and clean up when it does happen so the smell goes away.
