Sounds like an amateurish copy-cat of the Israeli pager operation, obviously tampered packaging and the device wasn't designed to be detonated remotely but just by activating the headset. If I was any sort of quartermaster I would be checking inside the casing of every electronic device upon receipt since the pager attack regardless of evidence of tampering.
This all seems fairly easily thwarted by adding testing to the role of quartermaster. The pipeline between acquisition and deployment to troops should include proper testing to be sure all equipment acquired from private companies is not tampered with.
Maybe add to that not just private companies but all equipment that travels outside of military delivery mechanisms, so that it can not be tampered with via agents in the postal/delivery sector which is very easy to get into. All you reasonably have to do is get a delivery driver job to do that kind of middle-man attack.
