My interpretation is it's a metaphor for people (ghouls) like Chuck Schumer who are doing absolutely nothing to stop Trump's worst offenses. I don't think it's literally attacking paramedics for making memes instead of treating patients.
Schumer has more than a passing resemblance to that comic character:
